The applicant should send the completed Application Form / Upgrade Form and photocopies, together with a crossed cheque made payable to "Hong Kong Institute of Medical Laboratory Sciences Ltd." covering admission fee and/or annual subscription of appropriate membership status, to the Registrar of the Institute, through the following addresses:

525 - 543 Nathan Road, Yaumatei, Kowloon, Hong Kong.

Please download the application form according to the categories of membership. Applicant should also send his/her recent photo (~5 cm x 4 cm) or email its digital image (JPEG format, 800 x 600) to info@hkimls.org for issue of new membership card. Replacement of membership card due to loss or damage will be charged at cost for HK$50 each.
